Dr. Andrew E. Nullman M.D.P.A, LLC d/b/a Andrew E. Nullman M.D. ("Dr. Andrew Nullman", "Dr. Nullman", "us", "we", or "our") operates http://www.drnullman.com (the "Site"). We are the controller for personal information submitted to us under this Policy. Dr. Andrew E. Nullman M.D takes its customers’ and consumers’ data privacy rights very seriously and has enacted this Policy to ensure those rights are maintained and protected. I understand that, under The Health Insurance Portability Accountability Act of 1996, I have certain rights to privacy in regards to my protected health information (PHI). I have received, read and understood The Notice of Privacy Practices.

I understand that I am formally giving consent
to the practice to receive pharmacy and medication information that will be received from any and all outside pharmacies, hospitals or other healthcare institutions. This information may be received electronically via electronic medical record or in writing.
I give permission to the physician and their staff to leave a message on my automated answering device or to family member regarding results of any test or appointments that were done in this office and/ or referred by this office.

Changes To This Privacy Policy
This Privacy Policy is effective as of June 1, 2020 and will remain in effect except with respect to any changes in its provisions in the future, which will be in effect immediately after being posted on this page. Our practice reserves the right to change the terms of the Notice of Privacy Practices. I understand the Practice will provide me with a copy of its Notice of Privacy Practices on request
